Case Manager
The primary responsibility of the Case Manager is to guide and empower patient/participants to play a major role in their health and wellness while facilitating the use of evidence based tools, standards and guidelines outlined by Aval Health Management.  
(Field based part-time and full-time opportunities)

Responsibilities:

• Provide patients with clear direction and support through one-on-one communication and coaching to promote healthcare consistent with their Physician’s care plan.
• Collaborate with healthcare team to facilitate cost-effective utilization of patient resources.
• Conduct participant assessments to establish program direction and goals.
• Identify and implement appropriate health intervention programs for outcomes reporting.

Qualifications:

• Current Registered Nurse (RN), Licensed Practical Nurse (LPN) and/or Registered Dietitian (RD) License required. Certified Diabetes Educator (CDE) Preferred.
• Minimum 3-5 years of recent experience in Case Management / Patient Care Coordination.
• Ability to work independently, as well as cooperatively with other team members.
• Excellent organizational, communication and computer skills required.
